Use of ECS Form used by MDIndia Health Services for making payment against your Medical Bill is compulsory for your Medical Bill for 2011 due to be sent now by 15th January alongwith other papers. Pl. also note that alongwith this a cheque of your Bank Ac. signed by you but cancelled also must accopany your papers and documents. In case you do not have a copy of this please let me know your need and e-mail id.
